For the safety of our community during the COVID-19 pandemic, Stephens College requires all students and employees to upload certain documentation to the Stephens Health Portal.
For the purposes of efficient contact tracing, both students and employees are required to submit positive COVID-19 test results within four hours of receiving results.

- Submit positive COVID-19 test result >
- Submit COVID-19 vaccination card >
- Submit weekly COVID-19 negative test result (if not fully vaccinated) >
- Submit other immunization records >
