Medical Excuse Policy
It is the policy of Stephens College Health Services not to provide a medical “excuse” for missed coursework to students seeking routine outpatient care. Excuses may be given in cases of serious illness, or in cases where a patient’s class attendance is a threat to public health, such as with communicable disease. If a student is advised to stay home from class, documentation will be given to the student.
Health Services will gladly provide a verification of visit form to all students who request it, stating the date and time of your appointment and, if you chose, your diagnosis.
Students should make efforts to notify instructors of the reason for their absence in a timely manner. Students should also seek medical attention during, not after their absence from class, if they intend to request a verification of visit form. Health Services will not grant retroactive verification of illness. Students and faculty should strive to resolve concerns that arise when illness interferes with academics with appropriate honesty and trust.
If student’s feel they have acted appropriately regarding an absence and their instructor requires further documentation, the student may contact Health Services for an appointment at 876-7157. Instructors are asked to use good judgment in requesting the use of Stephens College Health Services to verify absence for reasons of minor illness or in instances where a student had not consulted a health professional at the time of illness.
Immunization Policy
The required immunizations must be DATED as stipulated on the Health Statement. We will be happy to assist you in obtaining your immunization records upon your first visit to Health Services.
Health Statement Policy
The Health Statement must be filled out completely, including immunization and insurance information, prior to your date of entrance to Stephens College.
