Release of Information
Stephens College Health Services will not release your medical information to anyone, including family members, without your consent. Please fill out this form if you would like your parents to be able to access your medical information, or if you would like Health Services to send records to another medical facility.

Medical Records Request
Use this form if you would like Health Services to obtain records from another medical facility. Fill it out completely with the name of the health care provider in possession of the records, and bring it by Health Services to have it signed and faxed.
